ONEgeneration’s Case Management Services assists homebound older adults to age by providing services that will allow them to live independently and safely in their home, while preventing or delaying hospitalization and long-term placement.
When you or someone you know face new challenges that come with aging, assistance is available.
Eligibility Criteria for Case Management Services:
- Enrolled in Case Management
- Must be 60+ years of age
- Must be homebound – minimal local driving for medical reasons and essentials permitted
- Clients must live within our service areas.
If you live in Reseda, Winnetka*, Canoga Park*, Encino, Tarzana, Woodland Hills, West Hills call (818)708-6624 to confirm.
If you live in Northridge, Winnetka*, Canoga Park*, Porter Ranch, Chatsworth, Granada Hills, North Hills call (818)654-8379 to confirm.

- Case Management available in English and Spanish
- Client Evaluation & Assessment
- Information & Referral Services
- Daily hot meal Delivery (Monday through Friday)
- Emergency Alert Response Systems
